We are proud to announce that Beatrice Community Hospital has been recognized as an Age-Friendly Health System – Committed to Care Excellence.
The hospital announced its participation in this initiative in January and upon successful participation for the past three months, which included submitting data in the 4Ms areas, has gained full recognition as an Age-Friendly Health System – Committed to Care Excellence, an initiative of The John A. Hartford Foundation and the Institute for Healthcare Improvement (IHI) in partnership with the American Hospital Association (AHA) and the Catholic Health Association of the United States (CHA).
As a participant in the initiative, BCH Quality Coordinator Kayleigh Allen has been working with hospital case management staff, Acute Care nurses and Rehab therapists to fully implement evidence-based interventions specifically designed to improve care for older adults.
These interventions, known as the 4Ms, are:
“This program helps us to refine our processes and ensure that everyone is following the same best practices,” Kayleigh said. “The Age-Friendly Health Systems initiative is an important part of our vision to provide every older patient in our hospital with the best care possible and have the data to prove it.”
