The Social Services Department assists patients, family members and their healthcare team, understand and cope with the financial, social and emotional concerns that relate to their illness or hospitalization.
The Social Worker coordinates discharge planning for inpatients and works with the patient, family and medical staff to formulate a discharge plan that is appropriate for each situation.
Services
- Advanced Directives – Answers questions and provides assistance in filling out required forms
- Discharge Planning – Assists patients and their families with coordinating the resources necessary for a safe and healthy discharge from BCH
- Swing Bed Program – Contact person for other hospitals transferring patients to BCH Swing Bed program; provides required admissions information to these patients; participates in weekly Care Plan conference and communicates this information to the patient, family and staff; performs discharge planning
- Patient Representative for BCH grievances – Assists patients or their representatives in filing a grievance with the hospital.
