What is the Connect2MyCare Patient Portal?

The Connect2MyCare Patient Portal is a free, secure, personal gateway into your healthcare at Beatrice Community Hospital and Health Center and participating physician clinics.

The Connect2MyCare Patient Portal allows you to view parts of your electronic health record at your convenience from the comfort of your home or office.

The Connect2MyCare Patient Portal also allows you to add your own family health history and details so that it can become your own personalized and portable electronic health record.

Can I view health information from my physician clinic visit on the Connect2MyCare Patient Portal?

No, the Connect2MyCare Patient Portal is a free service.

Who can see my medical information in the Patient Portal?
Only you can see your medical information, and only you can give permission to others to view your Portal. Patients are encouraged to protect their medical information by not sharing their Username and Password. See (Patient Portal For My Family FAQ.)

Will I be able to pay my hospital bill online?

Yes. Once you have registered for the Connect2MyCare Patient Portal, you will automatically have access to any outstanding balances for which you are the primary guarantor.

If you have not had a service at the hospital since June 16, 2014, but would like to pay an outstanding balance online, please contact Beatrice Community Hospital and Health Center's Patient Financial Services Department for further assistance.

What do I need to access the Patient Portal?
--Access to a computer and the Internet.
--An email address. A unique email address is needed for each patient, for example, a husband and a wife will each need their own email address.

Any person 19 years of age or older can register for an account.

How do I register for the Connect2MyCare Patient Portal?

For Clinic Patients:
If you are already enrolled in the Connect2MyCare Patient Portal, and you receive a service at one of the hospital's participating clinics, you will be automatically linked to the clinic where you are having your service.

 

For all Patients who want to enroll in the Connect2MyCare Patient Portal:
Email Invitation:

1. If registering for a service at Beatrice Community Hospital and Health Center or a participating clinic, an invitation asking you to register for the Patient Portal will be sent to the personal email address you provided. Follow the directions in the email invitation to register for your account. Remember each patient must use a unique and personal email address. Two people cannot share an email address for purposes of the Patient Portal.

2. After you have completed the registration process, a message will be sent to the email address you provided indicating whether your registration was successful, and confirming your Username.

3. You may now log in to your personal Patient Portal page, and view parts of your health record. Health information about services provided at Beatrice Community Hospital and Health Center beginning in mid-June 2014 is avialable for viewing. Health information about services provided at participating clinics beginning Sept. 1, 2015, is available for viewing.

Self-registration:
1.
Go to Connect2MyCare.com. Click on "Register Here," and then click on the Patient icon. Fill in the registration form and click on the Register button.
2. After registering for your account, you must link to Beatrice Community Hospital and Health Center and the clinics of your choice in order to view your health record information, if available.

     To link, follow these next steps:
Step 1: From your Home screen, click on the "Providers" tab, and then click on "Add Provider or Facility."

Step 2: Enter the word "Beatrice" or the zip code and click Search.

Step 3: Click on "Select" next to each facility of your choice. Repeat steps 1-3 to link each family member.

Step 4: Within five business days, you will receive email notification(s) indicating whether you have been approved to link to your health record(s).

PATIENT PORTAL FOR MY FAMILY

Can I view another family member’s health information?
Yes. you can view the health information of a legal dependent. Proof of legal responsibility may be required, e.g., copy of Power of Attorney, Guardianship, Joint Custody, etc.

If you do add a dependent family member to your account, you are obligated by applicable laws and the RelayHealth Terms of Use to ensure that you are legally entitled to view the dependent patient's information and to make decisions about the collection, use and disclosure of the patient's personal health information. As the primary account holder, you have access to clinical information about the patient.

Please ensure that you have the appropriate legal right to do so by reviewing the "Patient/Authorized Representative Acknowledgment" pop-up and clicking "Yes, I confirm." You will not have access to the dependent's connected providers/facilities or any other health information added to the dependent's record by a connected provider/facility), until you do so.

For each dependent you wish to manage, you must go through the step of adding the family member and linking that family member to Beatrice Community Hospital and Health Center and the clinics of your choice.

I have never received a service at Beatrice Community Hospital and Health Center or one of its participating clinics, how do I request approval to view a dependent's health record information on the Connect2MyCare Patient Portal?

1. You must fill out a Proxy Access Form (available at the registration desk). You must return this form in person, along with one form of identification, such as a Driver's License, to the registration staff who will review and validate the information. For questions about the proxy application, please contact the registration desk staff of the facility where you are receiving your service.

2. After the Proxy Access Form has been validated by the registration staff, an invitation will be sent to the email address that you provided.

3. Click on the link in your email invitation to complete the registration process. After registering and opening your portal account, you will need to link yourself to the hospital and/or the clinics of your choice (use Add Provider or Facility tab). You will then need to add your dependent(s) to your account. To do this: click on the Providers tab, then click on Add Family Member tab, complete form and click save. And then you will need to link your dependent(s) to the hospital and/or clinics of your choice (use Add Provider or Facility Tab). Please allow up to five business days for your online request to be processed. Once the request is approved, you will be able to view your dependent's health record information.

What if I don’t see my medical information?
Health information should be viewable in your Patient Portal within 36 hours of hospital discharge or within four business days of your clinic visit.

Health information about services provided at Beatrice Community Hospital and Health Center beginning in mid-June 2014 is available for viewing on the Connect2MyCare Patient Portal.

Health information about services provided at participating clinics beginning Sept. 1, 2015, is avialable for viewing on the Connect2MyCare Patient Portal.

What if my demographic information is incorrect?

You can update your demographic information, i.e., address, phone, email, when you are viewing your Connect2My Care Patient Portal site. On your Home page, click on the Account tab to access your account information. However, to keep your hospital medical record up-to-date, you must share with the hospital registration staff any corrections or changes you make on the Patient Portal the next time you register at the hospital.

The information you add in the Patient Portal can be changed by you. If you are requesting a change to your health information that was documented by your healthcare provider, contact the Health Information Management Department at BCHHC.

Our Patient Portal uses the latest security measures to ensure that your medical information remains confidential between you, the hospital and your care team. Additionally, you will set your own Password to secure access to your Patient Portal. Please keep your Username and Password confidential.

How can I obtain a copy of my entire medical record?
Please contact the Health Information Management Department at Beatrice Community Hospital and Health Center to obtain a complete copy of your hospital medical record. You will be asked to sign a Release of Information Authorization form. There may be a charge for personal use of medical records.

What happens if I don’t have an email account or Internet Access?

Your physician will continue to communicate with you through phone calls or letters as they do today. You can obtain copies of your hospital medical record from the Health Information Management Department at BCHHC.

What happens if my email address changes?
You can change your email address by updating your Account information. On your Home page, click on the blue Account tab to access your Account information.

What do I do if I forget my Username or Password?
When you register, you provide us with an email address. That email address becomes your User ID, unless you choose to change it in your portal settings.

If you have forgotten your Username or your Password, simply click the "Forgot your User ID or Password?" link, and follow the simple and secure process to retrieve your sign-in information. Or call Relay Health Customer Support at 1-866-735-2963 for assistance.

What is Pay My Bill?
Pay My Bill is an option on the Beatrice Community Hospital and Health Center secure Connect2MyCare Patient Portal that allows you to view details of your hospital statement online and to pay your bill online. Your hospital statement reflects charges for services provided by the hospital, Southeast Nebraska Surgery and Beatrice Orthopaedics and Sports Medicine.

 

How do I sign up for Pay My Bill?
When you register for the Connect2MyCare Patient Portal, you will have secure 24/7 access to your hospital statement online by clicking on the Pay My Bill tab.

 

What information will be available in my electronic account?
You will be able to view some of the service details, insurance payment details, personal payments and monthly statements in your electronic account.

 

How can I see details of my account?
After clicking on the Pay My Bill tab in the Connect2MyCare Patient Portal, go to the Account Summary page and click on Current Balance.

 

How can I see what insurance has paid on my account?
After clicking on the Pay My Bill tab in the Connect2MyCare Patient Portal, go to the Account Summary page and click on Current Balance.

 

What if I disagree with what my insurance company paid on my account?
For questions regarding insurance payments, please contact your insurance company directly.

 

Can I have access to pay another family member’s account?
You will have access to any dependent family members’ accounts for whom you are the guarantor.

 

MAKING A PAYMENT

How long does it take for payments to appear on my account?
Payments should appear on your electronic account within three business days.

 

Is there any fee associated with paying my statement balance online?
There is no fee for paying your statement balance online.

Can I pay all my healthcare providers through my Connect2MyCare patient portal online bill service?
Both your paper statement and your online statement reflect charges for services provided by Beatrice Community Hospital and Health Center, Southeast Nebraska Surgery and Beatrice Orthopaedics and Sports Medicine.
